I understand that tattooing is permanent and that if I choose to have it removed, it may be expensive and leave scars.
I understand the risks of body art including but not limited to infection, scarring, difficulties in detecting melanoma, and allergic reactions to tattoo pigment, latex gloves, and antiseptics.
I understand that tattoo inks have not been approved by the FDA and that the health consequences of using these products are unknown.
I agree to immediately notify the artist in the event I feel lightheaded, dizzy, or faint before, during, or after the procedure.
I agree to follow all aftercare instructions provided by The Chamber Tattoo Studio.
I understand that refunds are not offered for services rendered.
All questions about the body art procedure have been answered to my satisfaction.
I indemnify and hold harmless The Chamber Tattoo Studio and its employees against any claims, expenses, damages, and liabilities arising from this procedure.
I wish to proceed with this body art procedure and I assume any and all risks that may arise.
I acknowledge that should any part of this document be construed as illegal, that part shall be void and the remainder held in full force.
